ManyTones uses a custom accessibility service to make most of this magic happen. When you first start the application, you'll be prompted to manually enable this service and be redirected to the appropriate page to do so. Like all accessibility services, your device may warn you this may expose sensitive data to what you're enabling. However, ManyTones does not use any of this data in any way, and what's more, ManyTones does not have the necessary permissions to access the Internet or the outside world, so you can rest assured any data it collects will not leave your device. Of course, if you are still uncomfortable with any of this, you are free not to use ManyTones.
