ODK Collect is a phone-based replacement for your paper forms with support for capturing geo-locations, images, audio clips, video clips and bar-codes, together with numeric and textual responses. It can evaluate complex logic to control the display of prompts and to enforce constraints on their responses; it also supports repeating groups of questions, and data collection in multiple languages.
ODK Collect is designed to operate out-of-contact with a cellular/wi-fi network during the data collection effort. Once back in network coverage, the filled-in forms can be copied off the device or submitted to a server (that you control) for analysis.
ODK Aggregate is a free server that can distribute form definitions to ODK Collect devices, accept filled-in forms and generate CSV files or publish the data into Google Spreadsheets, Google Fusion Tables and elsewhere. There is a robust community providing servers, services and support to organizations deploying ODK-based surveys.
