COMJUNTOS es una app que ayuda a las familias que tienen un niño o una niña con una enfermedad rara a transitar por las principales situaciones comunicativas con los profesionales de la salud. El hilo conductor son 7 retos que invitan a las familias a superar algunas de estas situaciones, como el día que les comunican el diagnóstico, cuando tienen una visita médica, les entregan un informe médico o deben realizar una prueba médica. Cada reto incluye un vídeo con testimonios reales de familias afectadas y de profesionales de la salud, un hipertexto que explica la situación y anticipa las dudas que puedan surgir, un diccionario básico de términos médicos, e información de consejos, recursos y enlaces.
Han participado lingüistas, psicólogos y médicos de la Universitat Pompeu Fabra, de la Universitat Oberta de Catalunya y del Área de Genética Clínica y Molecular y la Unidad de Enfermedades Raras del Hospital Vall d’Hebron; además de contar con la participación de la Federación Española de Enfermedades Raras (FEDER) y de diversas familias asociadas. Se ha elaborado en el marco del proyecto RercerCaixa JUNTOS.
