Have you tried to enter your date of birth??? Terrible implementation. You have manually swiped month by month, one by one for decades until you find it. Then repeat the process to confirm. Took 17 minutes just to enter it in. Then, you can only search for appointments for a Dr in one city/office. Don't find an appointment and want to check another, it forces you to repeat dob entry, insurance all over again. You can't type in the date or just have it remember the first entry? Another 17 minutes
