Telecare è l’applicazione che permette ai pazienti cronici in cura domiciliare di raccogliere e comunicare i dati dei parametri vitali ai medici curanti, in modo continuo e in tempo reale.

ATTIVATA DAL MEDICO

Telecare può essere attivata solo dietro indicazione del proprio medico per partecipare ad un programma di monitoraggio remoto della salute. L’app offre ai pazienti e ai caregiver un’interfaccia semplice e intuitiva per raccogliere e comunicare in tempo reale dati relativi a:
- Parametri vitali (es. pressione sanguigna, temperatura...);
- Aderenza alle terapie;
- Questionari sulla qualità della vita;

FACILE DA USARE

Il paziente e il caregiver accedono attraverso l’app ed inseriscono le rilevazioni manualmente o automaticamente mediante dispositivi connessi. L’applicazione è indicata anche per tutti gli utenti che hanno poca familiarità con gli strumenti tecnologici.

Principali funzionalità
- Acquisizione di misurazioni attraverso dispositivi medicali associabili via bluetooth a smartphone o tablet;
- Inserimento manuale dei parametri rilevati per mezzo di dispositivi non Bluetooth;
- Accesso allo storico delle misurazioni;
- Sistema di reminder e alert in caso di rilevazioni di parametri fuori range;
- Questionari clinici validati profilati per categorie di pazienti;
- Diario delle attività per la raccolta di dati e informazioni generali sul proprio stato di salute.
