Die ACHT-Nachsorge App kann ausschließlich von Teilnehmenden genutzt werden, die bei der AOK Bayern versichert sind und bei einem teilnehmenden Adipositaszentrum eingeschrieben wurden. ACHT; Adipositas Care & Health Therapy zur strukturierten, sektorenübergreifenden Versorgung nach bariatrisch-metabolischer Operation, begleitet den Nachsorgeprozess von Patientinnen und Patienten nach einer solchen Operation.
Über tägliche Dokumentation in der App behalten sowohl Patientinnen und Patienten als auch die behandelnden Ärztinnen und Ärzte sowie Therapeutinnen und Therapeuten Nachsorgetermine, Ernährung, Bewegung und das persönliche Befinden der Teilnehmenden im Blick. Die ACHT-Nachsorge App ist über eine digitale Patientenakte mit einem Patientenlotsen verbunden, der die Teilnehmenden individuell berät und betreut.
