Build, share and analyze forms in seconds — powered by AI, saved in your Google Forms.
AI Form for google docs is the fastest way to create surveys, quizzes, registration forms and feedback forms on iPhone and iPad. Teachers, restaurant and shop owners, event organizers, HR teams: describe what you need and start collecting answers.
DESCRIBE IT, THE AI WRITES IT
Type what you need — "satisfaction survey for my café: welcome, dishes, prices, atmosphere" — pick the type, length, tone and language, and the AI writes every question. Review the result, rename it, regenerate it or use it as is, then keep editing in the app.
SCAN A PAPER QUESTIONNAIRE
Point the camera at a printed questionnaire: the app reads the questions, the options, the checkboxes and the rating scales, and builds the form for you. Recognition runs on your device — the photo never leaves it.
A NATIVE EDITOR, NOT A WEB PAGE
Every Google Forms question type: multiple choice, checkboxes, dropdown, short and long answers, linear scale, rating, date, time, grids, file upload. Add images and descriptions to questions and options, embed YouTube videos, organize with sections and titles, drag to reorder, undo after a delete. Preview the responder page at any time.
QUIZZES AND GRADING
Turn any form into a quiz: set the correct answer and the points for each question, add feedback for right and wrong answers. Google Forms grades every submission.
DESIGN
Change the fonts, text sizes, colours and header image of your form and see the result in the preview.
SHARE ANYWHERE
Copy the link, show a fullscreen QR code on a screen or a poster, export a social card image for Instagram, X or LinkedIn, or print a PDF copy of the form.
SEE EVERY ANSWER AS A CHART
Pie and bar charts per question, individual responses, and one-tap export to CSV, Excel or Google Sheets. Close the form automatically after a number of responses or on a date.
ALL YOUR FORMS, ORGANIZED
Folders, search, rename, duplicate, move, trash — and open any of your existing Google Forms from its link.
TEMPLATES
Order form, job application, time-off request, contact information, customer and event feedback, assessment, quiz, exit ticket, worksheet, course evaluation, party invite, RSVP and more — 17 ready-made templates to start from.
MADE FOR IPHONE AND IPAD
Interface in English, French, Arabic, Portuguese (Brazil), Spanish (Mexico) and Japanese. AI generation works in dozens of languages.
PRIVACY
Your forms and responses live in your own Google Drive — the app never stores them on its servers. Scanning happens entirely on your device. AI generation requests are processed by Google's Generative Language API.
