Mediq Diacare

The app Mediq DiaCare can be used in two ways:
- Mediq DiaCare: when the diabetes patient and caregiver collaborate in the health program ‘Mediq DiaCare’. Data from devices measuring blood glucose levels is visualized and shared with the patient and caregiver, providing insights to both. Furthermore, the app allows consult preparation between patient and caregiver, improving the flow of information before and during consults.
- DiaCare app: when the DiaCare app is solely used by the diabetes patient, and not by their caregiver, the app visualizes data from devices measuring blood glucose levels so that the patient has insight in their health. Furthermore, the app supports keeping track of insulin levels, carbs and physical activity.
Mediq DiaCare
Mediq DiaCare is used as an additional tool within an existing therapeutic relationship between diabetic patients and their caregivers. Currently, this app and its connected platform are used by hospitals and for their patients. The use of DiaCare, approved by patient and caregiver, leads to the caregiver having secured access to the patient’s data. This enables the caregiver to monitor and support the patient from a distance with managing their condition.
If your hospital is using DiaCare as a service, your caregiver will be able to provide you with access to the app. The use of DiaCare will become an integral part of the therapeutic relationship between you and your caregiver.
DiaCare app
The free DiaCare app is intended to be used by diabetic patients. The app supports patients with managing their condition by being able to sync blood glucose levels from medical devices, and keeping track of insulin levels, carbs and physical activity.
Blood glucose levels can be extracted automatically from medical devices using Bluetooth. Other data can be added manually. The DiaCare app provides clean insight in this data through graphs and overviews. DiaCare provides you with this insight anytime, anywhere!
