• Find the nearest Urgent Care Center to your location. The app displays a map of the nearest center along with the address and hours of operation.
• Access your medical records, including pharmacy history and lab results:
o The date of the test is displayed along with the name of the requesting provider and test results.
o The date you filled your prescription, dose, and who wrote the prescription are displayed.
• Our Interdisciplinary Care Team (ICT) are ready to assist you with managing a disease, providing educational materials, or letting you know if you have any outstanding exams or procedure that need to be done. You can send a secure email to your ICT to get answers to non-urgent questions.
o Based on PCP recommendations, we can provide you Continuous health monitoring equipment like Glucometer, Blood Pressure, etc.
