YOUR APPOINTMENT BRIEF

Add the reason for your visit, up to three questions for your clinician, and optional context in your own words. Choose up to nine Apple Health metrics and a date range. Vital Report creates a PDF that shows:

• The immediately preceding period of the same length
• Changes between those periods

Your text is shown exactly as you wrote it. It never selects, orders, or interprets metrics.

Vital Report does not send health values, questions, notes, reports, exports, or recipient details to its analytics or subscription providers. Limited product and subscription analytics do not include this content. You choose if and where to share.
