A mobile guide for your health journey.

As part of their rehabilitation, we ask our patients to sign up to Your SMHS Mobile Care App and enroll in the programs specific to their health care needs.
The App is designed to support and empower our patients to get the most out of their care and rehabilitation.
The current Program available is: Spinal Cord Injury and Rehabilitation Support.
What you will find in the App:
1. Information & Education
• About spinal cord anatomy and injury
• About each component of rehab and recovery
• Education videos – to support your self-directed rehab activities

2. Your daily schedule
• Shows your scheduled rehab activities and locations
• Helps you plan your week

3. Goal Reflection
• A space to reflect on your fortnightly rehab goals
• Log questions and comments in preparation for your fortnightly care planning meeting
• You as a champion of your own rehab goals!

4. Account settings
• Your notification and program settings
• Privacy policy, disclaimer
• Contact information

More about the Information & Education
The App aims to provide education and resources to complement the 1:1 and group education sessions on the ward, and to meet the your individual needs. The info can also be helpful for significant others and carers.

You can browse and find the education programs via the Information section of the App (centre icon at the bottom of the home page).

We will also provide some educational content via the daily tasks on your Home page. We will send you new education programs as they become relevant to your care and your specific rehab journey.

Integration with Healthkit now available to sync your health data (if relevant)The education content includes text, images, and video content as well as links to other useful resources on the internet.
