I had the same problem when trying to use the App after installing it I received the message "There was an error processing your request". I emailed AZV and was told they made an adjustment in the software. They said to delete the App completely and install it again. Answer no when the App asks you if you've already installed it on another device. It worked for me.

Selecting the date of birth too difficult, have to scroll month by month, why not input by manual. So uninstalled.

AZV's own online website apparently fails regularly. My own doctor couldn't look up whether I was eligible for certain services or not and asked me if I had downloaded the app. Off course, I had not. Why would I download an app that forces me to: - install a lock screen which I do not want, and - register by entering personal information which I do not want on my phone? So, now I am forced by AZV who demands I install their app on my property and dictates I enter sensitive information I rather not

Unreliable, it doesn't open when needed. When reloading it goes into error mode, I have to keep removing and adding my card because the app says my info is not available.
