Few suggestions from my side, so that the development team can look into it - 1) the DOB calendar should be more handy. 2) After submission of the form, if we open the app then the same landing page or entry page get open. 3) Regards address box for same address a checkbox should be there but in app it's says to leave the line blank for same address. If there is a checkbox and the user ticked it then it will allow the same address to autopopulate. 4) The gallery option should be more handy.
