Fold Care

Fold Care helps you stay engaged in your health journey by making it easier to manage your care, communicate with your providers, join virtual visits, and track your progress over time.

Practices using Fold offer an optional wellness feature that lets you connect your smartphone or wearable device through Health Connect. Connecting a device is completely optional and is not required to use the app.

If you choose to connect a device, Fold reads the following data (read-only) and shows it to you and your care team so they can support your care plan. Each data type maps to a specific in-app feature:

• Resting heart rate — shown as a trend on your Vitals screen so you and your care team can monitor your cardiovascular status over time.
• Heart rate variability (HRV / RMSSD) — shown as a trend on your Vitals screen to help your care team understand your recovery, stress, and overall wellness.
• Heart rate — shown on your Vitals screen alongside resting heart rate and HRV.
• Steps and exercise sessions — shown in your activity history so you can review your workouts and daily movement.
• Sleep patterns — shown as sleep trends to support your overall wellness.
• Blood glucose — shown as a trend to support chronic condition monitoring.
• Oxygen saturation (SpO2) — shown as a trend to support your overall health monitoring.

Fold only requests the minimum Health Connect data types needed for the features you choose to use. All health data is read-only, handled securely, used only to support your care experience, and never used for advertising. You can change or revoke access at any time in your device settings.
