*UPDATE: form health fixed this issue today* am a current patient and all was working fine for about 3 weeks, then the app won't recognize my phone # or email, so I can't log in to the app. On the computer I was able to log in to confirm I'm still a patient with upcoming appts, and the scale readings are being recorded. Have reported the issue to Form via email and phone message three times - still waiting for a reply. The issue is how to reset my log in to the app? Hope I receive a reply.

This is a great program and very effective in helping you meet your goals. The app is easy to use. It allows for an easy way to communicate with the program and provides a quick method for you to lookup all the information they provide. Juila is great, knowledgeable and helps keep you accountable. I was able to meet my goal in 3 months and have changed the way I look at eating. It is definitely worth it!

I have lost a lot of weight. I feel better than I have in many years probably since I was in high school no joke. I was stuck in a cycle of very unhealthy eating on a daily basis for many years. This has helped me detox from the fast food addiction. Feeling better, feeling healthier, feeling lighter on my feet is totally worth it. Totally worth quitting all the garbage indefinitely.

What a horrible system all around. Expect to be charged an unknown amount even if insurance insists it's "covered" - only means you pay for a copay for a bad system. App has several points where you cannot proceed even when all options are filled out, only choice is to restart the asinine form filling again. No integration with existing apps, so if you are forced on here and already logging elsewhere you are SOL.

Horrible. I just downloaded the app and it keeps glitching. I cannot even log in. I don't even know how I am going to get started if I cannot log in.

Horrible company full of horrible money hungry people. The only way you will lose weight is by crying yourself to sleep instead of eating from how bad they make you feel. I don't understand how they are allowed to exist. They are nothing but scammers. Not a single professional among them. Everything they offer they stole from another company or app. They insist you use their app for all issues, but it never works. They made sure the payment worked though. Sure they paid for their good reviews.
