Description

infoFarma és una aplicació electrònica que conté la informació mínima necessària per poder fer un ús segur dels medicaments que s’administren en els centres de salut de la Gerència d’Atenció Primària i a la Comunitat Camp de Tarragona.

Neix com una evolució de les fitxes de medicaments que es van elaborar en la nostra gerència l’any 2012 per donar resposta als requeriments dels procediments sine qua non del model d’acreditació del Departament de Salut i als nombrosos incidents de seguretat de pacients notificats en el nostre territori.

L’objectiu d’infoFarma és fer més accessible, a través de les eines habituals de treball (ordinadors i telèfons mòbils), als professionals la informació sobre la utilització i la seguretat dels medicaments inclosos en el petitori de farmàcia d’ús intern en la nostra gerència d’atenció primària, així com els medicaments ambulatoris que s’administren en els centres de salut.

Es mostra un llistat ordenat alfabèticament per principi actiu, amb la població en qui està indicat el seu ús, recomanacions de seguretat relacionades amb la seva conservació o risc associat i, en aquells que ho requereixen, una taula de dosi i volum de dilució a administrar per kilogram de pes corporal. L’aplicació proporciona informació sobre efectes adversos i recomanacions de seguretat quan aquesta està disponible. Per a alguns medicaments, la informació pot no estar disponible.

El seu disseny i desenvolupament ha estat portat a terme per la Unitat de Tecnologies de la Informació i Comunicació territorial, la Unitat de Qualitat i Seguretat dels Pacients territorial i la Unitat de Farmàcia d’Atenció Primària.
