I found this site easy to use. I'd like it if the rest of my medical history could be included from Ontario Shores (2017 - 2019), Lake ridge Health from 1997 to 2019, and North Whitby Medical Center, Dr. John S. Lee, from 09/96 to 12/31/2025 (Medea). Other primary files exist at Northumberland Hills Hospital from 2018 to 2024, and Ottawa General Hospital, 3 campuses, 2022 - open. (My Chart)

I don't recognize the Dr. nor his place of business. No phone # and directions, town. He may have swapped identities with his brother

App used to work great. Now, it will not do anything. I log in, and it goes to a white screen, and that's all it does. Have tried multiple ways to attempt to log in, and the result is the same. By reading other reviews, it looks like this has been an issue for months.

The app has not been working for me. After I sign in, it only takes me to a blank white screen, and I cannot access anything. I had my username and password reset, but the same issue is still happening. This has been very frustrating because I need to be able to use the app for my healthcare information.

Worked well for about a year now it won't even let me log in all I get is a blank screen and there isn't another app I can use.
