The OneCare Mobile app has the tasks for each day that can be used for healthy living, falls prevention and chronic disease management. Our mobile app has the ability to take your vitals each day through iHealth Labs FDA approved integrated bluetooth devices. OneCare also has complete education and behavioral coaching to help you better understand chronic conditions. Our smart assessments can alert the participant's Care Team of potential problems.

When you complete a task for the day, you are awarded a score for that task based on if you completed it on time or not. We will send you a reminder via text or email if a task is coming up, depending if you are signed up to receive them.

This mobile app is completely integrated with our cloud based clinical platform that allow for connecting patients, health care professionals and family members to achieve a better quality of life. Real time data analytics, secure services, and patient engagement are all part of this integrated platform.


If you do not have any of these bluetooth devices, they are available at Best Buy and Walgreens, or you can just manually enter in data via the keyboard if you have a different brand of device.
