There is no way to simply upload a pretrained model. If I wanted to use the model I have already trained, I would need to retrain it using the app along with a Python script rather than just the Python script.

Unfortunately there is still some bug, at least on Android 12, One Plus 8T: after the first use the screen gets black and the app needs to be uninstalled and reinstalled. Anyway it's really great to have YOLOv5 on a smartphone, 5-stars as soon as bugs are solved!
