Description
PREDICT – Prevention & Early Delirium Identification Carer Toolkit is an evidence-based digital resource designed to support family carers of hospitalised older adults at risk of delirium. Delirium is a sudden change in thinking, awareness, or behaviour that commonly occurs during illness or hospitalisation. It is distressing for patients and carers and is associated with longer hospital stays and poorer outcomes. Many carers want to help but are unsure what signs to look for or how to raise concerns with healthcare staff.
PREDICT was co-designed with carers, clinicians, and researchers to provide clear, practical, and accessible guidance that empowers carers to play an active role in delirium prevention and early identification.
What PREDICT offers Plain-language explanations of delirium and why it matters Common early warning signs carers can notice Practical, non-medical strategies to help reduce delirium risk A validated delirium screening tool suitable for non-healthcare professionals Short videos and visual resources to support understanding Guidance on communicating concerns with hospital staff Content aligned with current evidence and best-practice care Designed for real-world use Easy to use on phones, tablets, and computers Accessible for different literacy and digital skill levels Supportive, reassuring, and carer-focused
Research evaluating PREDICT shows improvements in carer knowledge, confidence, and ability to participate in delirium prevention and early detection.
PREDICT does not provide medical diagnosis or treatment and does not replace professional healthcare. It supports partnerships between carers and healthcare teams, helping carers prevent delirium and recognise changes early and seek timely clinical support.
