THROMBOLYSIS
Complete alteplase and tenecteplase protocols with dosing calculators:
- Alteplase: 0.9 mg/kg IV (max 90mg) - bolus and infusion protocol
- Tenecteplase: 0.25 mg/kg IV bolus - AcT trial 2022, now preferred in many centres
- Eligibility criteria including wake-up stroke (WAKE-UP trial) and extended window (EXTEND trial)
- Absolute and relative contraindications with risk-benefit guidance
- Complications: symptomatic ICH reversal protocol, angioedema management

ANTIPLATELET THERAPY
- Aspirin 300mg acute loading - IST/CAST evidence

- Clopidogrel - NICE NG128 preferred long-term agent, CYP2C19 interaction guidance
