With the CCP Patient Portal app, you can:

• View your cardiovascular Profile
• Receive alerts
• View and schedule appointments
• Request refills
• Communicate with office staff for billing and general inquiries
• Share data securely and in real time with cardiovascular provider
• Take care of your loved ones
• Integrate with Apple HealthKit to retrieve daily exercise logs, sleep patterns and health data

Garbage app. It doesn't let you update any information and keeps throwing "alert: undefined" which is the most unhelpful popup ever. Someone listed me as assigned female at birth, and it took me forever to get into my account because I was assigned male at birth, identify as such, and why the hell would I think to pick another sex. The field for DOB doesn't specify that it needs slashes for it to recognize it, so I put in multiple variations of my DOB before I figured it out. Unintuitive.
