Description
Use the Yellow Card app to report suspected safety concerns involving healthcare products, including a side effect with a medicine or an adverse medical device incident.
The Yellow Card scheme is the UK system run by the MHRA* for collecting and monitoring information on suspected adverse reactions to all medicines including vaccines, blood factors and immunoglobulins, herbal medicines and homeopathic remedies, and all medical devices available on the UK market.
The Yellow Card app allows users to:
Report safety concerns about: > Suspected side effects to a medicine or vaccine (including herbal products and homeopathic remedies) > Adverse incidents involving a medical device (including software, apps and artificial intelligence) > Suspected defective medicines (a product that is not working properly or not to the manufacturer’s specifications) > Fake and/or unauthorised medicines > Blood products
Create an account: > View previously submitted reports > Update your reports > Create a watchlist for alerts to products of interest > Track new safety information published by the MHRA > View numbers of reports received by the MHRA
All reports received through the app or website can help the MHRA identify potential new concerns about a medicine, vaccine or medical device and act as an early warning for further investigation. The MHRA will review the product if necessary, and take action to minimise risk and maximise benefit to the patients.
*The Medicines and Healthcare products Regulatory Agency (MHRA) is an executive Agency of the Department of Health and Social Care; the MHRA protects and promotes public health and patient safety by ensuring that medicines, healthcare products and medical equipment are used safely and meet appropriate standards of safety, quality, performance and effectiveness.

I understand that there is not enough information about the government's yellow card scheme as the true figures are shown of deaths and side effects of the covid vaccine.

The app was updated in January 2026, so it seems that most complaints about it have been resolved. I managed to submit a report without any issues. I received confirmation both in the app and via email. Date input is now by a drop-down for year, month and day. Fields have picked lists, but free text is also possible.

Don't understand the negative reviews, this worked fine for me first try. Submitted a vaccine reaction report and got an email notification straight away.

Not for Individuals - very bizarre! When you try to use this app to report medicine side effects, the very first COMPULSORY question is about who you are. It provides a drop-down list of options. ALL are medical professionals! There is no option for patient/individual ie the poor sod having problems with the med. This is a BIG change for Yellow Card - and not a good one. I know from experience that a lot of GPs can't be bothered doing it, so to exclude patient reports is madness!

While I saved. It does not show sent. Also, we should not have to open account to do anything. Should be like NHS Covid app. This app is rubbish

This app could have been good, but why have date of birth field as a calendar (that can only be changed month-by month, with manual entry not allowed). That's the most ridiculous system I've ever seen and lazy programming. I gave up with COVID-19 vaccine my report after it took me two minutes just to go back five years. Please fix the date of birth field.
