This seems like it's just a way for an insurance broker to get your information, so they can send you a plan. I didn't even go through the steps because they want too much confidential information just to help me "find the right medical insurance plan" you don't need my medicare number you don't need my effective dates you don't need my doctor's name and address etc. just to help me find the right plan. I'm only speaking for myself, but this seems deceptive.

They want ALL your info before you even talk to anyone. They want your Medicare number on this first form, and you HAVE to provide it to go on. I uninstalled this app. Too suspicious.
