This Anxiety Test is designed to help you evaluate the severity of your anxiety with just seven simple questions. This app uses the Generalized Anxiety Questionnaire (GAD-7), an empirically-based, self-test questionnaire. It is also sensitive for other anxiety disorders as well such as specific phobia, panic disorder, and social phobia. These disorders generally respond positively to treatment, so it is important to seek treatment if you are concerned about your mental health.

If you want an analysis of your state of mind you either need a longer test, I personally favor this one British test I saw that was some 150 questions long, but even still using an online test isn't ideal. You can passively say everything is fine and a screen cannot tell the difference, same goes for just clicking the most severe answer every time. I'd say the only way to use something like this is had someone you know hold the phone and read the questions aloud to the test taker, and intemperate tones and body language when answers are given.
