Description
The free caring@home app supports health professionals and families/carers who are looking after a person who has chosen to be cared for, and to die at home, if possible.
What's New in the 2025 Version: This update includes resources from the Community Palliative Care Clinic Box. These new resources will support families and carers with: • Managing physical symptoms • Recognising dying • Understanding infusion devices • Providing practical care • Managing subcutaneous medicines
The new Prompts for End-of-Life Planning (PELP) Framework is also available to support high-quality care across all settings.
The app enables health professionals to easily view, download or print the suite of free and best practice caring@home resources for families/carers, health professionals and clinical services.
The resources enable health professionals to teach families and carers to provide practical care and help manage end-of-life symptoms, including safely giving subcutaneous medicines.
The app can be used offline, making it ideal for health professionals working in rural or remote parts of Australia without reliable internet coverage. Select carer resources have been translated into nine languages commonly spoken in Australia.
The app has been produced by Brisbane South Palliative Care Collaborative (BSPCC), Metro South Health as part of the caring@home project. caring@home is funded by the Australian Government and conducted by a consortium led by BSPCC. Visit www.caringathomeproject.com.au for more information.

This is a really handy app to help doctors and nurses to use the (wonderful) caring@home packs. So helpful when you are on the road and need to print something or show a family a video. Makes palliative care at home that bit easier. Thanks.
